January 26, 2010 - Applications for Funding Available

The Madison South Rotary Foundation will be considering requests for funding in the spring
of 2010. If your non-profit organization would like to submit a request to our Foundation, it
is helpful for us to have all requests in the enclosed format. Please attach proof of your
organization’s non-profit exempt 501 status. No grant distributions will be made without this
proof. Please complete this application form and return by mail no later than Friday, March
12, 2010
